| Description | The ADRC in Orange County began operations in December 2008 with funding under the California Community Choices federal grant. CalOptima, which is a County Operated Health System and a managed health care plan, facilitates the 3-way partnership between CalOptima, the Orange County Office on Aging and the Dayle McIntosh Center (an independent living center). This ADRC partnership is unique in as much as one of the partners is a managed health care plan with members who are eligible for Medi-Cal and Medicare. This offers up the opportunity to closely link the formal health care coverage with the various community-based support services. The I&A call center operated out of the county office on aging is the hub of the ADRC services. The most recent goal is the implementation of the Coleman Transition Intervention (CTI) offering transition coaching to patients transitioning from hospital to home. The Dayle McIntosh Center, among other ILC services, offers support to individuals transitioning from nursing facility to home. |
