| Description | San Francisco uses an outstation model to bring ADRC services and information to many neighborhoods and cultures. The most recent goal is outreach to people with disabilities ages 18-59. The ADRC is working on a multi-language flyer easy to use by consumers and providers. We are also implementing the Coleman Transition Intervention (CTI) offering transition coaching to patients transitioning from hospital to home. |
