Report Title - The Power of Integrated Information: Benefits and Challenged of Developing a Long Term Care Data Warehouse in California This report was commissioned under the California Community Choices Project to explore models and promising practices for better integrating long-term services and supports (LTSS) data. The survey and analysis of healthcare and other LTSS data collected across departments, environmental scan of lesson-learned and promising practices, identification of barriers to data integration, and the survey of assessment tools will be particularly beneficial as the State considers technical means for improving the delivery of services under the Coordinated Care Initiative.
