Strategic Plan
The California Community Choices Strategic Plan was developed with significant input from the Community Choices Advisory Committee, key state department representatives and the public. This plan will be used as a living document to guide and monitor implementation of the Community Choices project beginning July 2007. The Strategic Plan format is prescribed by the Centers for Medicare and Medicaid Services and used by all recipients of federal Real Choice Systems Change Grants.
